New Patient Information


New patients are welcome. Your first appointment will take roughly one hour during which we examine your mouth, head and neck - jaw joints, hard and soft tissues, muscles, and lymph nodes, as well as the teeth.

Radiographs (x-rays), impressions of the mouth, and photographs may be needed to assess dental conditions and plan for treatment. A personalized treatment plan and cost estimate is provided along with a time frame for completion of the service, in most cases, in writing.

You will need to arrive at least 15 minutes before your first scheduled appointment to fill out a medical history form and give the receptionist all your dental insurance information. If you wish to save some time, you can download and print out both the Medical History form and the Personal Info form and fill them out ahead of time. Non-Canadians should also please download, fill out and bring the liability disclaimer form to their first appointment.

Note: You will need a copy of the free Adobe Acrobat Reader to view and print these forms.

A second consultation appointment is often needed to discuss the proposed treatment prior to starting any work.

PapeFinch Dental offices are centrally located near downtown Toronto and in North Scarborough close to public transit lines and major highways.

Missed or Cancelled Appointments


Your appointment time is reserved only for you, thus a missed appointment means that another patient is unable to be treated. If you must reschedule your appointment, we ask that two-business days notice be given.

 

Payment Policy


PapeFinch Dental follows the current Ontario dental fee guide.  This fee guide is based upon clinic costs, laboratory costs, and professional fees. The fees are inclusive, however unforeseen costs such as the modification of treatment plan during treatment, increases in laboratory fees, delays in treatment, etc. may have an effect on total costs incurred.

Payment for all examinations, consultations, and treatment performed is expected at the time the services are rendered or initiated. Our offices accept most dental insurance plans.  We are happy to fill out your paperwork so that you can be reimbursed for any coverage that your plan may provide for dental services. We also accept cash, personal cheques and all major credit cards.

Most insurance companies base their reimbursement on the Ontario Dental Fee Guide.
